Removed email notification to "Facilitator" for new entries in the Activity Log.

Added ability to collect reasons for "employment termination" within the Career Journey and Staff Roster at the "Employment" level (instead of "Position").

Fixed "Configurator" role not being removed after updates to the individual's account.
